Gaps in the current rules

The light-mode status failures are untriaged. Whether they are known and accepted, scoped to non-text uses, or unnoticed is not recorded. See TITAN-DIV-03.
text-error-invert on background-error fails in dark (2.49:1) and is marginal in light (4.36:1), despite the names implying they pair.
No token expresses dark text on a bold status fill, which is what conformant light-mode success and warning banners require.
No semantic focus token exists. See TITAN-DIV-02.
No reduced-motion implementation exists in the token layer. See TITAN-A11Y-06 below, and Motion.
No forced-colors / High Contrast Mode guidance exists.
Focus Not Obscured is unaddressed. No rule governs the interaction between the z-* ladder and focus.
Contrast is checked here, not in Titan — see TITAN-A11Y-01 and TITAN-A11Y-03 below.

Constraints Titan should enforce

These are real, currently-true rules. Nothing in code stops a builder from violating them today — each is a placeholder for a guardrail Titan hasn’t built, not a decision still being made.

TITAN-A11Y-01	Text meets 4.5:1 (or 3:1 for large text) contrast on every surface it appears on.	Extending `emit-tokens.mjs --check` to fail the build when a token’s contrast against its documented surface(s) falls below threshold, instead of only reporting it.
TITAN-A11Y-02	Status text and icons use the `-alt` token variant.	A lint rule flagging non-`-alt` status tokens used as a `color`/`fill` value.
TITAN-A11Y-03	Contrast is checked against `background-view-background`, not only `background-default`.	The same build-time contrast gate as TITAN-A11Y-01, checked against both surface columns.
TITAN-A11Y-04	Focus is never removed without a replacement style.	A stylelint rule requiring a replacement declaration alongside any `outline: none`.
TITAN-A11Y-06	Every transition respects `prefers-reduced-motion`.	Shipping the `@media (prefers-reduced-motion: reduce)` reset as a global stylesheet in the token package, rather than leaving each consumer to add it.
TITAN-A11Y-07	Interactive targets are at least 24×24px, including an icon-only control’s width.	Defaulting an icon-only control’s min-width to its `control-height` token value.
TITAN-A11Y-08	Opacity is never used to derive a color variant.	A lint rule banning direct reference to any `*Channel` token outside the token build.
